::By that definition, has there ever been a fatal rocket crash (excluding rockets fired as weapons hitting their target)? Thinking of the US space program: Apollo 1 was a fire in the capsule on the ground, not a crash. Challenger was an explosion in mid air, not a crash. Columbia was a break up on re-entry, not a crash.
